Student Health Care Plans
Students with a diagnosed health care need that requires attention and/or monitoring at school are required to have a health care plan on file. These health care plans need to be signed by the parent or guardian as well as the treating medical professional. These must be updated and completed every school year.
Here are some commonly used student health care plan forms to provide to your primary care provider. Once completed, please turn into your student's school office.
Meal Modification
If your student has a food allergy or intolerance that would require them to receive different breakfast or lunch options in the cafeteria, a signed medical statement for meal modification is required.
This only needs to be completed ONCE as long as your student attends a Colorado school.
This MUST by signed by one of the state-approved medical providers.
Approved signatories are: MD, DO, NP, or PA. Chiropractor, RN, LPN, or Holistic providers are not approved by the state.
Examples of food allergies or intolerance that would require this form may include but are not limited to:
Lactose/dairy allergy, Celiac disease, Peanut allergy, Fish/Seafood allergy.
Medications at School
If your student needs prescribed or over-the-counter medications regularly or even for a short-term need, a signed medication order and permission is required. Medications must be FDA approved in order to be given at school. Any over-the-counter medications (such as Acetaminophen, Ibuprofen, Benadryl, etc.) that need to be given for longer than two weeks or differently than the package directions will require a physician signature.
Students in grades 7 and up are allowed to self-carry medications with a signed permission and agreement on file. Students in grades 4-6 may be approved on a case by case basis.
