Student Vaccinations

Vaccinations are an important part of school readiness which keeps children from spreading diseases that can make themselves and others sick. Colorado law requires students who attend public and private schools to be vaccinated against certain diseases a certain number of times. These required vaccines are:

  • Diphtheria, tetanus, and pertussis (DtaP, Tdap)
  • Hepatitis B
  • Measles, Mumps, and Rubella (MMR)
  • Polio (IPV)
  • Varicella (Chickenpox)

Kindergarten & Middle School Requirements
  • Kindergarten students MUST receive their FINAL doses of DTaP, IPV, MMR & Varicella or provide one of the exemption options before the first day of school.
  • Students entering 6th grade MUST receive one dose of Tdap (given after the 10th birthday) or provide one of the exemption options before the first day of school.

WHAT THE SCHOOL NEEDS FROM YOU

In order to avoid lost learning time, please complete one of the below options to make sure your student is allowed to attend school:

  1. Provide a complete record of up to date vaccinations to the school
    • Provide a Certificate of Medical Exemption SIGNED BY AN APPROVED MEDICAL PROVIDER
    • Provide proof you have completed the online exemption module in order to receive a Non-Medical Exemption (Must be done EVERY year)

Certificates for Medical Provider Signature and the Annual Online Education Module are available at: cdphe.colorado.gov/vaccine-exemptions

Tips for Completing the Annual Online Education Module:
    • Do NOT complete this on a phone, please use a desktop computer.
    • The school will provide you with access to a computer if needed.
    • Please print the certificate of completion or email a full screenshot.
    • These expire every year on June 30th and must be completed every year.

School Exclusions

If we do not have the required immunization information for your child by the first day of school:

  • We will call you and send home a notice, giving you 30 days to complete one of the above options.
  • If we do not receive anything from you within 30 days, we will call you and send home a school exclusion notice with a deadline date.
  • If we do not receive anything from you by the deadline date, your child will be excluded from school until we receive either a complete record, an appointment date, or an exemption.

If there is a vaccine-preventable illness or outbreak within the school district and your student is not vaccinated against the specific illness, they may be required by law to remain home from school. Depending on the illness this time period can be 21 days or more, even if a certificate of exemption is on file.

Questions

If you have questions about vaccinations or if you need help with medical transportation, or finding free or low-cost vaccines, please go to COVax4Kids.org or contact the District Nurse.

The Importance of Vaccination Schedules ...
How’s Your School Doing on Vaccinations?

Some parents, especially those with students who have weakened immune systems, may want to know which schools have the highest percentage of vaccinated students. Schools must report immunization and exemption numbers to the state health department annually. Student names and information are not released with this information. Schools do not control their specific immunization and exemption rates or establish the Vaccinated Children Standard described in §25-4-911, CRS. Schools must include their MMR immunization and exemption rates from the most recently completed school year in this letter. Schools may choose to also include immunization and exemption rates for other school-required vaccines. Additional immunization and exemption rates can be found at COVaxRates.org.

Below are the most recent immunization rate letters for each school within the Archuleta School District.
