Well Child Visits

 

A Well Child Visit is a regular check-up with your child’s doctor to make sure they are healthy and growing the way they should. During this visit, the doctor will check your child’s height, weight, hearing, vision, and more. They also talk about important things like sleep, eating healthy, staying active, and keeping up with school. Your child may also get vaccines to help protect them from serious illnesses.

 

Well Child Visits are important because they help catch problems early, before they become bigger issues. These visits are a chance to ask questions about your child’s health, learning, or behavior. When children stay healthy, they are able to focus better in school and feel more confident. As a school nurse, I encourage all families to schedule these visits every year—even if your child feels fine—because staying healthy helps them do their best in school and in life.

 

If you are experiencing issues getting connected with a local medical provider, affording health care, or getting transportation to your child's medical appointments please contact the district nurse. 

Sports Physicals
 
Sports Physicals are separate and not a substitution for recommended well child visits. 
Most local medical offices offer these specific visit types at a reduced cash price but they do require appointments. 
Each medical office will have paperwork specific to their office, but below is the required CHSAA physical evaluation form.
